Duration of the visit in months is counted in days. Take a look at the following configuration:

Another issue that might happen. You need to add custom validation to the study edit page: duration of the visit must be shorter than min(time difference between patient visits, time difference between control visits). Don't forget to include units for the follow up increamentals.

Next problem: add appointment button should be disabled for finished visits.

It should also be disabled in the visit details page.

When I create a new visit and define beginning of the visit I think it would be nice to autofill the end of the visit based on the default visit length (it was working that way before). Now the visit end must be filled manually:
