The analysis of the procedural memory is particularly relevant in neurodegenerative disorders like Parkinson’s disease, due to the central role of the striatum in procedural memory. It has already been shown that the anterograde procedural memory, the ability to learn a new skill, is affected in Parkinsons disease. However, to our knowledge, the functioning of retrograde procedural memory, the very long-term retention and execution of skills, has not yet been studied in Parkinson’s disease. Therefore, we will here, first, describe the development of a novel tool based on the Cube Copying Test to assess this memory concept, and, second, present evidence that the functioning of the retrograde procedural memory is indeed impaired in Parkinson’s disease and explore the associations of cube drawing performance with patient and disease characteristics.

Since there are no evidence-based protocols available to evaluate the retrograde part of the procedural memory, we developed a novel tool based on the Cube Copying Test. We established an extended evaluation system that evaluates both the drawing procedure, representing the retrograde procedural memory, and the final result, representing the visuo-constructive aspects.

A pilot study in 40 healthy subjects suggested four typical ways to draw the cube. To assess the role of retrograde procedural memory in Parkinson’s disease, a total of 272 participants from the Luxembourg Parkinson’s Study were classified according to the previously established patterns to draw a cube. Comparing people with typical Parkinson’s disease (n = 136) with age- and gender-matched control subjects (n = 136), we identified a significantly impaired cube drawing performance in people with Parkinson’s disease (p < 0.001).

Our study demonstrates an impairment of the retrograde procedural memory in Parkinson’s disease. The extended evaluation system could give further insights into the impairment of retrograde procedural memory in other cohorts of patients with Parkinson’s disease or other neurodegenerative diseases.
